For a provider contact to qualify for BHAG, the provider contact must be Director, C-Level or a physician, AND one of the following stipulations for the provider organization that the provider contact is associated with:

  • The provider category is an IDN.
  • The provider type is any other type except for Clinic or Other.
  • (Clinic or Other) The bed size is > or = to 20.
  • (Clinic or Other) The provider physician count is > or = to 11. The exception is for OB/GYN, Orthopedics, Oncology, Imaging which only requires the physician count to be > or = to 5.

BHAG for evaluations are counted on the date when the survey first hits Executive Review.
